Set Your Appointment

Call (915) 533-5600 during normal business hours. Please advise our staff if you are being referred or have had a change in your insurance policy. Please have the following information available: Full name, date of birth, insurance information, and preferred pharmacy.

Office Hours: Monday - Friday - 9am to 5pm

Payment / Insurance Information

 Please have your insurance card available while making your appointment. Please advise us of any of the following: Subsidiary Medicare program, Secondary Insurance policy, or Out-of-State policy. Our office staff will contact your insurance provider to verify all benefits. However, it is the patient’s responsibility to make sure that their policy is active and to be aware of any limitations of coverage.

We encourage you to save time by printing and completing the following on-line forms prior to your appointment.  Every new patient is required to read the “Notice of Privacy Practice” brochure.  Each patient must fill out the HIPPA Acknowledgement form, Patient Information Sheet, and Medical Questionaire. Additionally Medicare patients must fill out the Advanced Beneficiary Notice (ABN).

Bring Any Medications & Previous Records

It is very important for all new patients, referred patients, or patients that have seen Dr. Farnam at another practice location to obtain all pertinent records prior to your appointment.  You are welcome to have your records faxed to our office at (915) 533-5604.  However to ensure the availability of your information during your appointment and to prevent a delay in your visit, we recommend that you obtain a printed copy of your records and bring it with you to your appointment.  Please also bring any medications you are taking.
